Click “Get Qualified” to take our free 2-minute quiz or talk to your doctor and obtain a letter of medical necessity for weight loss coaching
Request an Itemized Receipt from Us
Click on the “Send a Receipt” button and complete the form. We will email you an itemized receipt within 24 business hours
Submit documents to HSA/FSA Provider
Submit your itemized receipt and your letter of medical necessity to your HSA/FSA provider via mail or their portal
